The Social Worker is responsible for the provision and coordination of support services to patients and their families. The Social Worker needs a wide variety of interpersonal and communication skills to develop effective working relationships with patients, families, staff and other professionals. He/She must advocate for the patient and organize/implement referrals that will best suit the patients needs. The Social Worker will know of and have access to a wide variety of resources that will assist the patient and family in their admission to the long term care facility and discharge to the home environment. He/She will collaborate with other team members to assure that all support services are in place in the inpatient facility and prior to discharge.

The ideal Social Worker has a Bachelor's Degree in Social work, one year experience in the field, and an upbeat and engaging personality.

The following is a representative list of duties and responsibilities associated with this position:
-- Assess patient's psychosocial needs, and assist in the development of a treatment plan to meet the needs
-- Visit residents and develop a supportive relationship with residents as well as their families
-- Provide supportive counseling to residents and their families as needed
-- Assist in referring patients to community and outpatient medical services which will benefit the resident 
-- Provide information to service providers to ensure coordination of services, and establish effective working relationships with medical providers and service agencies 
-- Participate in the interdisciplinary planning process, and attend meetings as necessary
-- Documentation of services to include progress, declines, and changes of status 
-- Ensure care plans are complete and in compliance with Federal, State, and Local regulations
-- Develop and produce reports regarding services and needs as requested and required 
-- Assist in the completion of the Minimum Data Set and assessment protocol reviews as needed for each patient
-- Provide administrative case management services to patients, which will include assisting patients and their families to complete necessary paperwork
-- Coordinate the admission process
-- Coordinate discharge planning process
-- Attend in-service, staff development/training events, and applicable meetings as appropriate
-- Perform other duties as assigned to ensure the delivery of a comprehensive Social Service program
-- Knowledgeable in effective counseling techniques; behaviors and issues of long term care patients and effective interventions; community skills and working with families
  Bachelor's degree in Social Work
  One year experience in Social Work
  Ability to enter documentation of services provided into computer system
  Able to problem solve and work effectively with others
  Demonstrated proficiency at educating, communicating and empathizing with patients, answering questions and concerns in a manner that is respectful is required
  Extensive network of relevant contacts among industry professionals and trade associations required
  Communication and interpersonal skills to develop relationships with clients, staff and providers
  Excellent verbal and written skills
  Genuine desire to work with the long term care/rehabilitation population
